PDF to Excel - Contact List

Completed a project from a client having 1 page with some names and contact details to be entered into a spreadsheet(Either an Excel .CSV or .XLSX file). Data is entered into different columns for Name, Title, Company, Street Address, City, State, ZIP, Phone, Fax, Email, Website. Adding only the a screenshot of the project here for better visibility.
